High Profile Action Request Form (HPARF)

New Information:

• Form Link Updated.
• Process: PTS > ACTION > Tick HPARF when filling form.
• Contact a member of the NSC with any questions.
• If you are reporting a member of the NSC, the only recipient is the DepSecDef (to be contacted via Forums using the template below).
• The HPARF selection under the ARF Form is only used to report JSD+ and/or OJSIG. If you would like to file a complaint towards all other personnel, do not select HPARF when filling the form linked in Table 1.
